The plantar fascia is a tough band of fibrous tissue running from the heel bone to the toes. It supports the arch of the foot from below and absorbs some of the impact with every step. Many people worry when an X-ray shows a heel spur. A heel spur is simply a sign of prolonged tensile stress on the bone — its size alone does not confirm it as the current pain source. The examination focuses instead on tenderness location, first-step pain pattern, and the response to dorsiflexing (bending back) the toes. Whether a spur is visible or not, the physical examination takes precedence over imaging.

Once an attachment-site problem is confirmed, the first priority is reducing the load on the foot that same day: adjusting walking distance, changing footwear, and deciding together how far to push calf and plantar fascia stretching.

Is the Pain on the Inner Heel, or Somewhere Else?

Press firmly on the inner underside of the heel, close to where the big toe side begins. Clear tenderness there — especially with the first step in the morning or after a long rest — points to the plantar fascia attachment. Dorsiflexing (pulling back) the toes to tighten the fascia and checking whether that reproduces the pain is part of the standard exam.

A bruised, deep ache directly under the center of the heel that worsens when stepping barefoot onto a hard floor suggests a fat pad problem. The heel fat pad is the cushioning layer beneath the heel bone. When this is the issue, aggressive plantar fascia stretching is less helpful; adding heel cushioning and reducing barefoot time on hard surfaces is a better starting point.

Deep, bone-level pain that worsens after a sudden increase in activity — or tenderness when squeezing both sides of the heel — raises concern for a stress fracture. If bearing weight feels unbearable, stretching and exercise should stop immediately, and imaging should be done to check for bone injury. Heel pain breaks down differently depending on where it is and what provokes it: the plantar fascia, fat pad, bone, and Achilles tendon each produce a distinct pattern.

If discomfort centers behind the heel rather than underneath it, and worsens when rising on tiptoe or climbing stairs, Achilles tendinopathy is more likely. The Achilles attachment at the heel bone is examined separately, and whether friction from the shoe's heel counter makes it worse helps inform the diagnosis.

Limited ankle dorsiflexion (the ability to bend the foot upward toward the shin) causes the heel to lift early during walking, which increases the pull on the plantar fascia. Examining tenderness location, toe-dorsiflexion response, ankle range of motion, and how much the foot collapses inward during walking together clarifies which structure is overloaded. Can Plantar Fasciitis Cause Numbness and Burning in the Sole?

Numbness, a burning sensation, and reduced feeling in the sole do not fit the typical presentation of plantar fascia pain. When these symptoms are present, identifying which nerve is compressed or injured takes priority over continuing treatments aimed at the fascia.

An electric-shock-like tingling on the inner ankle that spreads into the sole suggests tarsal tunnel syndrome — compression of the tibial nerve (the nerve running along the inside of the ankle) within the narrow tarsal tunnel. Lightly tapping over that tunnel and feeling a tingling shoot into the sole is a positive sign, and the tunnel is then examined closely. Even compression of a single small nerve branch that passes along the inner heel can cause a burning sensation during prolonged standing — in a location that differs clearly from fascia pain.

Nerve compression in the lower back can produce pulling or tingling that travels from the low back or buttock down through the calf and into the sole. If coughing makes the sensation shoot into the foot, or if toe strength drops noticeably, examining the foot alone risks missing the real source.

Bilateral numbness that starts at the toes and moves upward like a sock in a person with diabetes raises concern for diabetic peripheral neuropathy. Burning that is worse at night, or difficulty sensing small wounds and temperature changes, warrants a careful inspection of the plantar skin. Any wound found there requires pressure relief first, alongside optimizing diabetes management.

Running and long walks that aggravate symptoms should be reduced for the time being, and switching to shoes with adequate cushioning and a stable heel counter is part of that adjustment. Calf stretching is performed in two positions: knee straight and knee slightly bent. For the plantar fascia, sit down, pull the toes toward you, and stretch only to the point where the sole feels taut. Corticosteroid injection — commonly called a "cortisone shot" — may be considered to shorten a period of pain severe enough to prevent normal walking. Repeated corticosteroid injections, however, may weaken the plantar fascia (raising the risk of a tear) or thin the protective heel fat pad. The number of prior injections is always asked about, and the current fascia thickness and integrity are reviewed on ultrasound before deciding whether another injection is appropriate.

Manual therapy is added for patients whose ankle stiffness causes the heel to rise early during gait, or who have developed an outward-leaning walk to avoid pain. The physician first assesses ankle range of motion and gait pattern, determines which areas to address and at what intensity, and the therapist then works one-on-one to restore ankle mobility and release calf tension.

How Long Does Recovery Take?

Plantar fasciitis tends to improve gradually with consistent conservative care. Resting for a few days alone rarely makes it disappear. Long-standing symptoms combined with a weight-bearing occupation, tight calves, worn-out shoe cushioning, and unmanaged activity levels all slow recovery.

Tracking progress by daily activities gives clearer feedback than counting days. Notice whether the morning limp lasts a shorter time, whether you can now walk the same distance without stopping when you used to have to pause just outside your door, and whether discomfort at the end of the workday is still lingering the next morning. These three markers guide gradual increases in walking distance and exercise intensity.

Stopping stretching and load management as soon as pain improves leaves the shortened calf and altered gait unchanged, so when a long day of walking or standing returns, force concentrates on the same attachment site and symptoms recur. Footwear and activity volume need to be adjusted together as daily activity increases.

Some symptoms should not wait. Persistent numbness or reduced sensation — or numbness in both feet in a person with diabetes — warrants nerve testing, and any difficulty sensing wounds means the plantar skin should be examined that same day.

Sudden inability to bear weight on the heel, or bone-deep pain that worsens after ramping up activity, requires evaluation for a stress fracture or acute injury. Prolonged pain and swelling after an ankle sprain, or noticeable weakness in the toes, should not simply be observed and attributed to plantar fasciitis without further workup.

The appropriate tests and the right time to seek evaluation differ depending on whether the problem is isolated inner heel pain on the first step in the morning, or whether numbness and reduced sensation in the sole are also present.
